The hospital payment system is complex in Romania. Romanian hospitals receive monthly prospective payments, as defined in the contract with the District Health Insurance House (DHIH). The total contracted amount is calculated by using a mix of qualitative and quantitative (including DRG, case-payment, and fee-for-service payment mechanisms) factors. In addition, certain hospitals can receive lump sums per patient for complex and expensive procedures via the National Curative Programs.

National Health Insurance House (CNAS) and the Ministry of Health (MoH) are key reimbursement stakeholders that develop the Framework Contract, which defines the guaranteed services, typically every two years.
